How do I become a patient of North Star?

<< Back to Common Questions

If you are an expectant mom, we encourage you to take advantage of our get-acquainted visits while you still have the time - and energy - to do so. It’s a good idea to try to meet the pediatrician that will be caring for your new baby so you can be sure your styles and personalities mesh. But if you don’t have the opportunity, don’t worry; it’s certainly not required. When you check into the hospital to deliver, just let the staff know who your pediatrician will be and they’ll do the rest. If you deliver at Community Hospital North, we will be automatically notified and will visit you and the new baby within twenty-four hours of birth. If you deliver at another hospital, which often happens, the hospital nurses or our office staff can suggest another pediatrician who will care for your baby during the hospital stay. When you are discharged home, call our office to schedule your first check-up, usually around day 5 of life, or 2 to 3 days after leaving the hospital.

If you have older children and plan on bringing them to our office, just call when you are ready for their next appointment, whether it is a sick visit or a check-up. After you decide to join our practice, you’ll need to notify your prior physician that you would like your child’s medical records transferred to us. While it is helpful to have your child’s old records on hand to review, especially if your first visit is a well-child visit, it’s not required.

There are several forms that will be filled out at your first visit. Downloadable versions are available in the Information for Patients section if you would like to fill them out prior to your appointment.
